What is the Medicare GUIDE Program?
What is the Medicare GUIDE Program?
The Medicare GUIDE Program is a national initiative created to help individuals living with dementia and the caregivers supporting them access coordinated care, resources, and respite services while staying at home.
This program is designed to support your current doctors, not replace them. It adds structured dementia support, navigation, and caregiver assistance to help reduce stress and prevent avoidable health crises.
WHO QUALIFIES?
You May Qualify If Your Loved One:
- Has traditional Medicare (not Medicare Advantage)
- Has a diagnosis of memory impairment / dementia
- Does not live in a nursing home
- Is not enrolled in hospice (palliative care is allowed)
- Dual eligibility with Medicaid or VA is allowed (if Medicare is primary).
What Families Receive Through GUIDE
This program includes support services designed to reduce caregiver burnout and improve safety at home:
1) Respite Care Coverage
Up to $2,563/year in covered respite services that may be used for:
· Private duty home care
· Adult day services
· 24-hour community respite care
2) Care Navigation + Monthly Support
Families receive ongoing support that can include:
· Caregiver training
· Resource referrals
· Telehealth coordination
· Emotional + logistical guidance
3) Medical Support (Telehealth Dementia Visits)
The program includes telehealth dementia-related care visits that complement your existing provider team.
