How to Request Home Care Services from the VA Clinic

We are committed to those who have served our country and recognize that every veteran has unique experiences, preferences, and needs. That’s why we take a personalized approach to care. Our team works closely with each veteran and their family to create a customized care plan that addresses their specific requirements and goals. We adapt our services as needs change, ensuring our veterans receive the highest level of care at all times.
What to Ask For:
If you are currently a patient at the VA Clinic in your area, you can request a “Homemaker or Home Health Aide” to assist you with activities of daily living, or “respite care” to provide relief for a family caregiver. If you are not currently a patient, you will need to apply to the clinic and be seen by a physician before you are able to ask for home health services through the clinic.
The Steps:
- Call the Social Services Department at the VA Clinic and ask to apply for a Home Health Aide.
- If you are eligible, the VA Social Worker will complete and submit the application for approval.
- Once approved, ask the VA Social Worker to send the referral to CareBuilders at Home.
- Once the referral is received, we will contact you to schedule an in home assessment.
- Services can begin 24-48 hours after the assessment based on the number of service hours approved by the clinic.
